How Premier Health & Wellness Center collects, uses and protects personal information — both the clinical records we hold as your physicians, and the much smaller amount of information this website handles.

Premier Health & Wellness Center operates in Bermuda. Personal information we hold is governed by Bermuda's Personal Information Protection Act 2016 (PIPA), together with the confidentiality obligations that bind us as registered medical practitioners.
We also hold ourselves to the safeguards set out in the United States' Health Insurance Portability and Accountability Act (HIPAA) as a matter of internal standard. HIPAA is US legislation and does not itself govern a Bermudian clinic; where the two differ, PIPA and Bermudian medical confidentiality rules take precedence, and nothing in this notice reduces the rights PIPA gives you.
Clinical records. When you are a patient, we hold the information needed to care for you: your medical history, examination findings, laboratory and diagnostic results, medications, correspondence with other clinicians, and the notes made at your appointments.
Contact and administrative information. Your name, address, telephone number, email address, date of birth, and the details needed to arrange appointments and settle accounts, including insurance details where you use them.
Information you send through this website. The appointment and referral forms on this site collect what you type into them and nothing more.

As with any request to an outside server, these providers will see your IP address and basic browser information. Our web host also keeps standard server logs. None of it is combined with your clinical record.
Please do not send clinical details by email or through a web form in an emergency. Web forms are not a monitored emergency channel. If your concern is urgent, telephone the clinic on (441) 292-5111, and in an emergency call 911.
We use Google Analytics to understand which pages people read and how they arrived — so we know what is useful and what is not. It sets cookies, and it sends data to Google.
Nothing is set unless you agree. The first time you visit, a bar at the foot of the page asks. If you decline, no analytics cookie is set and your browser never contacts Google at all — we do not load the script rather than loading it in a restricted mode. Your choice is remembered on your own device; clearing your browser data will bring the question back.
If you accept, Google receives your IP address (which Google Analytics truncates), the pages you view, roughly where in the world you are, and basic device and browser information. We have disabled Google’s advertising signals, so this is not used to build an advertising profile of you. We do not sell it, and it is never joined to your medical record or to anything you enter into a form.
The site sets no other cookies of its own. Nothing you read here is used to target you with advertising, on this site or anywhere else.
To change your mind, clear this site’s data in your browser settings and the question will be asked again. You can also block analytics cookies entirely in your browser, or use Google’s own opt-out add-on.
We use your information to provide and coordinate your care, to arrange appointments and follow-up, to communicate with other clinicians involved in your treatment, to bill you or your insurer, and to meet our legal and professional obligations.
We do not sell personal information. We do not share your clinical information for marketing purposes.
Under PIPA you may ask us to confirm what personal information we hold about you and to provide a copy of it; ask us to correct information that is wrong or incomplete; ask us to stop using it for a particular purpose; and withdraw a consent you previously gave, where our use rests on that consent.
Some limits apply. We must keep medical records for the period required by Bermudian law and our professional obligations, so a request to erase a clinical record cannot always be granted. Where we cannot act on a request we will tell you why.
To make a request, contact the clinic using the details below. We may need to verify your identity before releasing health information. If you are not satisfied with our response, you may complain to the Office of the Privacy Commissioner for Bermuda.
Access to clinical records is limited to staff who need it to do their work. This website is served over an encrypted connection, and form submissions are transmitted over encrypted connections to the provider named above. No system is perfectly secure, but we take the protection of health information seriously and review our arrangements as our services change.
If we change how we handle personal information, we will update this page and change the date shown at the top.
