We Treat the Drivers.
We Measure the Change.

Type 2 diabetes, hypertension, and metabolic syndrome respond to care that targets their biological drivers — insulin resistance, inflammation and metabolic function — alongside medication where it is clinically indicated, with your results measured at every stage.

"The evidence is clear that the biological drivers of these conditions can be changed. Our work is to measure them, treat them directly, and show patients the difference in their own results."

The Biology Driving Metabolic Disease

Type 2 diabetes, hypertension, and metabolic syndrome are linked by a shared underlying biology: insulin resistance, chronic inflammation, visceral adiposity, and impaired mitochondrial function. Our physicians work on those drivers directly — through precision nutrition, movement, and structured metabolic monitoring — while prescribing medication where it is clinically indicated and adjusting doses, including reducing them where a patient’s markers and clinical picture support that, under physician supervision. Where a patient has been referred to us by another physician, that changes: we recommend adjustments in writing, with our reasoning and a monitoring plan, and the referring physician makes the prescribing decision.

For patients, this means a lifetime of medication titration, a gradual acceptance of progression, and the psychological burden of a condition defined as incurable. The clinical and financial implications of this model are considerable — and the evidence increasingly points to an alternative.

What the Science of Metabolic Reversal Shows

Over the past decade, landmark clinical trials have demonstrated that structured, physician-led intervention — precision nutrition, metabolic monitoring, and behavioural protocol — can achieve sustained remission in Type 2 diabetes in a significant proportion of patients. The DiRECT trial, published in The Lancet, reported remission in 46% of participants at 12 months — rising to 86% among those who lost 15 kg or more. The Virta Health studies demonstrated sustained reversal at five years.

The biology is now well established: when the drivers of insulin resistance are addressed directly, the body's metabolic machinery can restore normal function. PHWC's protocols are designed around this evidence — applying it in a physician-supervised clinical context, with the monitoring rigour and personalised adjustment that successful outcomes require.

Clinical Programmes

Two Pathways. One Goal.

Choose the programme matched to your clinical profile — or combine both for comprehensive metabolic restoration.

Diabetes & Metabolism

Sugar Restore

Type 2 Diabetes & Metabolic Syndrome

Our Sugar Restore programme targets the root cause of Type 2 diabetes: insulin resistance. Using precision nutrition, metabolic mapping, and continuous glucose monitoring, we work to restore insulin sensitivity and achieve A1c normalisation — with the goal of medication remission.

  • Comprehensive metabolic baseline (A1c, fasting insulin, HOMA-IR)
  • CGM monitoring and glucose pattern analysis
  • Precision nutrition protocol designed to your biology
  • Medication review and structured reduction pathway
  • Monthly physician check-ins with biomarker target tracking
Baseline A1c, fasting insulin and HOMA-IR, retested through the programme
Medication reduced only where your markers and clinical picture support it
Cardiovascular Health

Cardio Restore

Hypertension & Cardiovascular Risk

The Cardio Restore pathway uses advanced biomarker profiling and carotid IMT imaging to assess true cardiovascular risk — not just blood pressure numbers. We build a personalised intervention plan targeting the specific drivers of your risk.

  • Advanced lipid panel (Lp(a), ApoB, oxidised LDL)
  • Carotid IMT ultrasound scanning
  • 10-year cardiovascular risk stratification model
  • Targeted lifestyle and medication optimisation protocol
  • Quarterly biomarker monitoring with protocol adjustment
Risk assessed on carotid imaging and advanced lipids, not blood pressure alone
10-year risk recalculated at each quarterly biomarker review
Evidence-Based

Why Reversal is Possible.

The medical literature on Type 2 diabetes remission has grown dramatically over the past decade. Multiple landmark trials — including the DiRECT trial and the Virta Health studies — demonstrate that structured clinical intervention can achieve sustained remission in a significant proportion of patients.

46%

of participants in the DiRECT trial achieved remission at 12 months — rising to 86% among those who lost 15 kg or more (Lean et al., The Lancet, 2018)

5 years

of sustained Type 2 diabetes reversal reported in the Virta Health studies

The Protocol

A Structured Path to Reversal.

Step 01 Biological Baseline Assessment

We start with a comprehensive panel: A1c, fasting insulin, HOMA-IR, full lipid profile, inflammatory markers, and hormone panel. This tells us exactly where your biology is today — and what specific mechanisms are driving your condition.

Step 02 Personalised Clinical Protocol

Your physician designs a precision programme — nutrition, movement, sleep, and if necessary, medication optimisation — based on your specific markers, not a generic template. Every aspect of the protocol is explained, documented, and co-created with you.

Step 03 Active Monitoring & Adjustment

Monthly or quarterly physician reviews. CGM data reviewed. Markers tracked. Protocol adjusted in real time as your biology responds. We don't set a plan and disappear — we stay engaged throughout the entire reversal journey.

Step 04 Outcomes & Consolidation

Once targets are achieved, we shift to a maintenance protocol — sustaining remission or target levels while monitoring for any regression. For patients who achieve full medication remission, we continue annual biomarker reviews to maintain long-term results.

What Our Patients Say.

"My A1c went from 9.2 to 6.8 in four months. I came off two of my three diabetes medications. I genuinely feel like I got my life back."

M.T.
Hamilton, Bermuda

"I had been told my blood pressure was just something I'd live with forever. Within 8 months at PHWC my BP was normal and I had halved my medication dose."

D.W.
Warwick, Bermuda
